Home Health RN-{169713} Location: Vidalia, GA 30474 (Onsite, field-based home health) · Coverage counties include: Johnson, Emanuel, Toombs, Montgomery, Tatnall, Evans, Candler, Jeff Davis, Treutlen Pay Rate: $56.14/hour · Mileage Reimbursement: $0.43 per mile Schedule: Monday to Friday, 8:00am-5:00pm On-Call: Once a month for one week Overview We are hiring a Home Health RN (Case Manager) to support patients throughout the Vidalia, GA area. In this role, you will complete in-home visits, perform assessments, coordinate care, and ensure members receive the right level of support and services. Key Responsibilities Perform comprehensive assessments for high-risk patients/members Develop and help implement individualized care plans with providers Coordinate care transitions with physicians, social workers, discharge planners, etc. Reassess care needs and ensure cost-effective, appropriate levels of care Support compliance with treatment plans by helping coordinate non-medical resources (housing, transportation, etc.) Maintain accurate documentation of clinical, functional, and outcome data Productivity Expectations Minimum 30 points per week Typical volume: 4 to 6 patients per day and 20 to 25 patients per week Required Skills & Qualifications Active RN license (GA or compact) 1+ year home health experience (required) OASIS & Homecare Homebase (HCHB) experience (required) Strong clinical home health skills, including: Wound vac, PICC line care, Ostomy care, IV antibiotics, Wound care Preferred Case Management certification (if applicable/available) Keywords (for search) Home Health RN, RN Case Manager, OASIS, HCHB, Homecare Homebase, Wound Care, Wound Vac, PICC, Ostomy, IV Antibiotics, Care Coordination, Field Nurse, Skilled Visits
