We are seeking a driven, relationship-focused Community Liaison to join our Solace Hospice team. In this role, you will build and maintain strong partnerships with hospitals, skilled nursing facilities, assisted living communities, physicians, and community organizations to increase awareness of Solace Hospice and support appropriate hospice referrals. You will be a trusted resource—educating referral partners and families on hospice eligibility, services, and the Solace difference—while collaborating closely with our clinical and intake teams to ensure a smooth referral-to-admission experience.
This position is primarily community-based (“on the road”) and ideal for someone who is energized by networking, service, and helping families access compassionate end-of-life care.
Responsibilities
Develop and maintain referral relationships with key community partners including hospitals, SNFs, assisted living, physicians, home health agencies, case managers, and community organizations.
Educate referral sources on hospice philosophy, eligibility guidelines, levels of care, and Solace Hospice service offerings.
Identify opportunities for growth within assigned territory and execute a consistent plan for outreach, follow-up, and relationship development.
Serve as a liaison between referral partners and Solace Hospice, ensuring timely communication and excellent service.
Coordinate with intake, nursing, and leadership to facilitate a smooth admission process and remove barriers to hospice access.
Participate in community events, professional meetings, and networking opportunities to increase brand visibility.
Provide in-services and educational presentations for facilities and provider groups.
Maintain accurate activity documentation, referral tracking, and pipeline reporting in CRM/EMR tools as assigned.
Ensure all marketing and referral activities align with hospice regulations, HIPAA, and ethical standards.
Support patient and family experience by responding promptly, communicating clearly, and representing Solace with warmth and professionalism.
Skills
Candidates should possess a combination of required skills and preferred experience, including but not limited to:
Strong relationship-building, communication, and presentation skills.
Sales, business development, or healthcare marketing experience (hospice experience strongly preferred).
Understanding of hospice, home health, SNF, assisted living, and discharge planning workflows.
Ability to educate clinical and non-clinical audiences with confidence and compassion.
Excellent organization, time management, and follow-through in a field-based role.
Proficiency with CRM/EMR tools, Microsoft Office/Google Workspace, and mobile communication.
Valid driver’s license, reliable transportation, and ability to travel locally daily.
Professional judgment, emotional maturity, and comfort discussing end-of-life services with sensitivity.
Preferred (not required):
Existing relationships within the local senior care/healthcare referral community.
Knowledge of hospice eligibility basics (LCDs), levels of care, and payer mix (Medicare/Medicaid/commercial).
Job Type: Full-time
Pay: $75,000.00 - $90,000.00 per year
