Our mission is to provide high-quality, convenient, and patient-centered care in the comfort of patients' homes. ⸻ Job Description:We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our team on a part-time basis. The provider will conduct in-home visits, delivering comprehensive medical care to a diverse patient population. ⸻ Responsibilities: Perform in-home patient evaluations and physical examsDiagnose and manage acute and chronic conditionsPrescribe medications and treatments as appropriateDocument visits accurately in the electronic medical recordCoordinate care with physicians, nurses, and other healthcare professionalsProvide patient and family education ⸻ Requirements: Active and unrestricted NP or PA license in New York StateBoard certification (NP or PA)DEA registration (or ability to obtain)Prior experience in primary care, geriatrics, or home-based care preferredStrong clinical and communication skillsReliable transportation and willingness to travel within Suffolk County ⸻ What We Offer: Competitive pay at $150/hour (Two patients per hour. $75.00 per patient)Flexible schedulingSupportive and collaborative team environmentOpportunity to make a meaningful impact in patients' lives ⸻ How to Apply:Please submit your resume and a brief cover letter outlining your experience and availability Company DescriptionPlease note that this is a fully on the road job.
