Scope Of ResponsibilityThe Case Manager will work as part of our Early Intervention Day Treatment Program providing mental health services to children ages 2.5-5 and their families/caregivers.Essential FunctionsUtilize trauma-informed interventions in a therapeutic milieu-based programWork as part of a multi-disciplinary team providing therapeutic and psychoeducation services to children and familiesUtilizes Handle with Care interventions to manage crisis situations and high risk behaviorsAdvocate for parents and children when making outside referrals for servicesDevelops and provides client rehabilitation services which assist in improving, maintaining, or restoring daily functioning at home, school, and community in accordance with treatment goals and in collaboration with treatment teams (i.e., therapist, psychiatrist) following Department of Mental Health contract or other agency guidelines.Provides interagency and intra-agency consultation, communication, coordination and referralsTimely completion of documentation including progress notes using an Electronic Health Record SystemMeets or exceed service delivery requirementsRide Wellnest transportation vans to support children during transportation to and from the program.Represents the Organization at community services, funder events and events related to outreach for clients and supporters of the organizationOther duties as assigned Required QualificationsBachelor’s Degree in social work, psychology, or other mental health related discipline. Bilingual Spanish ability highly preferred, but not required. Employee must maintain a valid California drivers license and clean driving record that meets Wellnest’s insurance carriers’ criteria for liability coverage. Applicant is required to drive in order to make home, school, job site and other community-based visits. In keeping with Company policy for all employees, the successful applicant must also complete a test for TB, and fingerprint clearance by the Department of Justice.Working ConditionsEmployee regularly needs to work out in the field, in a variety of environments. QualificationsCOMPENSATION AND BENEFITSAnnualized Salary Range: $48,664 to $58,290 This position is also eligible for an annual $3,000 bilingual Spanish differential Wellnest offers $10,000/year to build a personalized health benefits package from a choice of available medical, dental, and vision coverage plans, as well as an employer-paid $50,000 life insurance policy.
