This position focuses on preventive measures and visits for Medicare patients along with patient outreach for assistance with medications and social determinants of health.You will also be responsible for follow up of patients who have recently been hospitalized or in a long term care facility, such as a nursing home.Benefits include health/dental/vision/life, paid holidays, PTO, employee discounts and much more.Must be up to date on licensure and certifications including BLS.Experience in phlebotomy, IV therapy and quality measures is a plus.
