Nurse - Patient Aligned Care Team - Care Manager (Deltona)

The PACT Care Manager RN is responsible for the care of Veterans with acute and/or chronic medical, surgical, and/or mental health conditions. The RN must provide a focused assessment, critical reasoning, and astute clinical judgement in order to expedite appropriate care and treatment, especially given that the patient may present with complex problems or potentially life-threatening conditions and/or behavioral health emergencies.The RN is a proficient nurse who understands a situation as a whole and perceives its meaning in terms of long-term goals. The proficient nurse learns from experience what typical events to expect in a given situation and how plans need to be modified in response to these events. The RN demonstrates performance and leadership in delivering and improving holistic care through collaborative strategies with others. They are self-directed and have a global understanding of how their area(s) of responsibility support the system's mission and vision. They demonstrate sound decision-making and judgment and possess excellent interpersonal and collaborative skills.The registered nurse provides care and counseling of persons to promote and maintain health and the prevention of illness and injury based upon the nursing process, which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being, and executing medical regimens, including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community.
