Experienced Nurse Practitioner / Physician Assistant - In-Home Acute Care
How You'll Make An ImpactWe are seeking experienced Nurse Practitioners and Physician Assistants with acute care backgrounds in ER, Urgent Care, or Internal Medicine to join our dynamic team serving the Kansas City, KS community. In this role, you’ll collaborate with Medical Technicians to deliver comprehensive healthcare in the comfort of patients’ homes. This includes everything from common injuries to more complex conditions through health assessments, diagnosis, treatment, and outcome management for patients of all ages.Our innovative care model allows us to bring advanced medical care directly to patients' homes, including point-of-care lab work, minor procedures, splinting, wound care, suturing, and IV administration. By facilitating timely follow-up and coordinating care, we provide holistic, patient-centered care.What You'll DoCollaborate with Medical Technicians and other team members to provide in-home patient care using our innovative service model.Travel to patients’ locations in a mobile unit, providing comprehensive health assessments, diagnoses, and treatments for complex illnesses.Deliver therapeutic interventions such as splinting, suturing, wound care, and minor procedures.Use critical thinking skills and follow evidence-based medical standards.Thoroughly document patient encounters with accuracy and attention to detail.Analyze test data to diagnose and arrange appropriate follow-up care for continuity.Educate patients and families on managing acute illnesses and overall health.Navigate sensitive conversations about end-of-life issues and goals of care with compassion.Communicate effectively with patients, families, medical power of attorneys, and other healthcare providers.Proactively identify and resolve clinical issues.Adhere to clinical and safety standards, protocols, and performance metrics.Provide care with empathy, cultural sensitivity, and professionalism.Foster strong relationships with team members and remote partners.Attend training sessions and clinical meetings to stay up-to-date with best practices.Lead by example, always putting the patient's well-being first.What You'll NeedExperience: A minimum of two years as an Advanced Practice Provider (APP) in Emergency Medicine, Urgent Care, Internal Medicine, or related fields (hospital, family practice, cardiac, pulmonary, etc.).Qualifications:Advanced degree from an accredited institutionCurrent, unrestricted state license as a Nurse Practitioner or Physician AssistantNational board certification (current)Current BLS certification (ACLS preferred)Prescriptive authority and DEA certificationValid driver's license with a clean driving recordPhysical Requirements:Ability to lift and carry up to 50 poundsComfortable walking up and down several flights of stairs while carrying equipmentAvailability to work flexible hours, including evenings, holidays, and weekendsExperience as a paramedic, RN (ED/ICU), Flight Nurse, or relevant graduate from an ENP program or fellowship is a plus.Who We AreWe are redefining healthcare delivery through mobile and virtual healthcare solutions.
