At CWS, compassion, integrity, and collaboration guide everything we do, and we invite you to join us in shaping the future of healthcare together Job Title: LPN – Skilled Nursing Facility (SNF) Job Location: Trinity Homes, Minot, ND Working Hours: 2×12-hour shifts + 2×8-hour shifts per week Shift: Days – 06:00 AM to 6:30 PM Department: Skilled Nursing Position Summary The Licensed Practical Nurse (LPN) at Trinity Homes provides high-quality nursing care in a Skilled Nursing Facility setting. The role includes supporting residents with daily clinical needs, performing assessments, administering medications, and collaborating with the interdisciplinary care team. The ideal candidate is dependable, patient-centered, and comfortable working in a fast-paced SNF environment with rotating responsibilities. Key ResponsibilitiesDeliver safe and effective nursing care to SNF residents.Perform assessments, monitor conditions, and communicate changes to the RN or provider.Administer medications, treatments, and wound care as ordered.Assist with ADLs, mobility support, and resident comfort measures.Document all care accurately in the EMR (Cerner).Work collaboratively with nursing staff, physicians, and support teams.Maintain infection control and safety protocols.Provide support across assigned units as needed during 12-hour and 8-hour shifts. Required Qualifications & SkillsActive LPN license (ND license accepted upon offer).Minimum experience requirement: less than 1 year acceptable.SNF or long-term care experience preferred.Cerner EMR experience preferred.Strong clinical judgment and time-management abilities.Ability to work 12-hour and 8-hour rotations, including holiday coverage.Excellent communication and patient-care skills.
