Job Description Job ResponsibilitiesThe role involves delivering direct patient care within the Primary Care department. You will also provide clinical support to Physician Assistants and other team members as needed.Engagement in interdisciplinary team efforts is essential to ensure the highest quality of patient care is either maintained or enhanced. You will take part in initiatives aimed at improving service delivery, which may involve program participation within the facility. Regular evaluations of patients will be conducted to monitor any changes in their physical and mental health, with progress notes documented in their medical records.Your responsibilities will include coordinating with healthcare providers both within your service line and across other divisions, including CBOC Clinics, as outlined in the veteran's care plan. Maintaining open, clear, and collaborative communication is crucial. You will be accountable for minimizing barriers for both Veterans and staff to foster a positive work atmosphere. Building strong relationships with colleagues, peers, and Veterans is a key aspect of this role.You will perform patient assessments, order and interpret diagnostic tests, including blood and fluid analyses and imaging studies, as necessary. Medical care will be provided as appropriate, ensuring that all staff in Primary Care are properly credentialed and privileged.Participation in various hospital committees, departmental in-service programs, and educational initiatives is expected. You will also engage in continuous quality improvement efforts through peer reviews and the FPPE/OPPE process. Active involvement in compliance readiness activities, as requested by the Chief of Primary Care, Quality Management, and the Chief of Staff, is also part of your duties.
