Pathwell Health - The RN is responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required.ESSENTIAL DUTIES AND RESPONSIBILITIESThe essential functions include, but are not limited to the following:Provide skilled nursing treatments as ordered by the client's doctorComplete evaluation tasks, including vital signs and medication reviewsCoordinate with physicians, therapists and other individuals in the client's care planAssess and chart observations of the client's condition at each visitMaintain complete and accurate documentation of nursing careWork collaboratively with all members of the health care teamProvide education to clients and families on proper home health care procedures and strategiesRecord patients' medical information and vital signsMonitor, record, and report symptoms or changes in patients' conditionsAdminister local, inhalation, intravenous, or other anestheticPrepare patients for and assist with examinations or treatmentsModify patient treatment plans as indicated by patients' responses and conditionsOrder, interpret, and evaluate diagnostic tests to identify and assess patient's conditionMonitor all aspects of patient care, including diet and physical activityEngage in research activities and participation in formal educational training activities related to nursingPerform other duties as assignedMINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)Associates degree required; Graduate of an accredited state approved, Bachelor's Degree from school of nursing preferredPRN Role availableCurrent and valid Connecticut Registered Nurse LicensePrior nursing experience, specifically in home health (preferred)Current CPR certification (required)Driver's License, Car Insurance, Car RegistrationStrong interpersonal skills that build personal rapport and establish trust with the clientComputer literateStrong organizational skillsExcellent written and communication skillsAbility to meet physical demands of the job (ex: lifting 50 pounds for transfers)Home health care experienceBilingual (Spanish) preferredPHYSICAL DEMANDS AND WORK ENVIRONMENTThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
