Registered Nurse- Community Care Healthcare Coordinator

This position supports the planning, coordination, and oversight of program operations within the Miami VA Healthcare System. The role ensures efficient workflow, adherence to policy, and high-quality service delivery. The incumbent collaborates with interdisciplinary teams, assists with data and reporting needs, and contributes to performance improvement initiatives.The Healthcare Coordinator serves as a professional nurse responsible for leading, coordinating, and evaluating complex patient care programs across the Medical Center. The incumbent functions as a clinical expert, program coordinator, and resource for interdisciplinary teams, ensuring that patient care delivery is efficient, evidence-based, and aligned with VA directives and organizational goals. The Nurse demonstrates advanced clinical judgment, leadership, and systems thinking to improve outcomes for veterans served by the Miami VA Healthcare System.Duties include, but are not limited to:Serving as a program coordinator for assigned clinical or administrative care programs, ensuring compliance with VHA policies, standards, and accreditation requirements.Providing consultation and expert guidance to interdisciplinary teams regarding complex patient care issues and program operations.Collecting, analyzing, and reporting program data to support performance improvement, quality initiatives, and regulatory compliance.Developing, revising, and implementing evidence-based policies, procedures, and workflows to optimize patient care services.Coordinating communication, referrals, and transitions of care across clinics, services, and community providers.Leading performance improvement initiatives aimed at enhancing patient safety, efficiency, and veteran satisfaction.Delivering education, training, and support to nursing staff and other clinical personnel.Serving as a liaison between clinical services, leadership, and external stakeholders to support program operations and patient-centered care.Participating in strategic planning, committee work, and organizational initiatives aligned with the mission of the Miami VA Healthcare System.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Monday-Friday 0800am to 4:30pmTelework: Not EligibleVirtual: This is not a virtual position.Relocation/Recruitment Incentives: Not AuthorizedPermanent Change of Station (PCS): Not Authorized
