In this home-based care role, you'll partner closely with patients, families, and interdisciplinary teams to promote safe, effective, and compassionate care for elders and adults with disabilities. You'll guide patients through clinical coaching, care planning, and resource coordination-helping improve health outcomes and enhance quality of life.Job specificationCandidate requirementsJob specificationConduct in-person home visits to assess patient safety, monitor changes in health, and provide clinical coaching-including medication reviews.Implement and monitor individualized care plans, offering guidance and education via our digital care app.Coordinate appropriate referrals to community and clinical resources.Establish strong, trust-based relationships with patients and caregivers to promote engagement and reduce caregiver fatigue.Document assessments, interventions, and patient outcomes in accordance with regulatory and agency requirements.Participate in team meetings and contribute to projects and initiatives as needed.
