Outreach & Enrollment Coordinator-PACE

PACE, a member of Empath Health is seeking a meticulous and goal-driven Enrollment & Outreach Coordinatorto join our interdisciplinary PACE team in Bradenton, FL. In this role, the PACE Enrollment & Outreach Coordinator accountable for coordinating events in the community and in the centers, providing tours within the center for potential participants, families and caregivers, assisting the Outreach and Enrollment with various tasks including, scheduling the PACE eligibility and enrollment intake process, creating outreach flyers for community and in-center events.
