Medical Social Worker
Human Touch Home Healthcare is seeking a compassionate and dedicated PRN Medical Social Worker to join our interdisciplinary care team in Colorado. The Medical Social Worker is responsible for implementing standards of care for medical social work services in accordance with the state Medical Social Work Practice Act and supporting patients and families in navigating psychosocial, emotional, and environmental challenges related to illness.PAY is Per Visit ResponsibilitiesAssess the psychosocial status of patients related to illness and environment and communicate findings to the Registered NurseConduct social evaluations and develop individualized intervention plans based on identified needsMaintain accurate and timely clinical records, progress notes, and documentation for all assigned patientsProvide information, referral services, and resource coordination for patients, families, and caregiversServe as a liaison between patients/families and community agencies to support continuity of careMaintain collaborative working relationships with clinical staff to support coordinated patient careDevelop and maintain relationships with public and private community agencies as referral resourcesParticipate in interdisciplinary care planning, case conferences, and discharge planning activitiesAssist physicians and care team members in understanding psychosocial and emotional factors impacting patient healthPerform additional duties as assigned by the Clinical Supervisor or Clinical DirectorQualificationsBachelor's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE); Master's or Doctoral degree preferredMinimum of 1 year of experience in a healthcare settingHome health experience strongly preferredStrong verbal and written communication, documentation, and organizational skillsCurrent CPR certification (or ability to obtain upon hire)Valid driver's license, reliable transportation, and proof of automobile insurance in accordance with company requirementsBenefits: (Only for Full-Time Employees)Competitive salary commensurate with experience.Opportunities for professional development and career advancement.Positive and supportive work environmentContribution to improving healthcare access and quality in the communityComprehensive benefits package including:Health insuranceVisionDentalVacation TimeSick LeaveJob Types: PRNPay: $75 to $90 per visitMedical specialties: Home HealthWork setting: In-personAbility to commute: Colorado Springs, CO: Reliably commute (Required)Experience: Occupational Therapist and Home Health 1 year (Preferred)License/Certification: Occupational Therapist License (Required)The above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job.
