Join our team as a PACE Primary Care Physician and help older adults live independently while receiving high-quality, all-inclusive care.What You'll Do- Serve as primary care physician for PACE participants (55+)- Provide preventative, acute, and chronic care with a focus on aging with dignity- Collaborate with the Interdisciplinary team to coordinate holistic care plans.
