About the job Home Health Occupational Therapist - JacksonWe are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have experience in various settings, including inpatient, outpatient, and school environments. This role involves working with diverse populations, including pediatrics and geriatrics, to help patients achieve their maximum potential in daily activities. The Occupational Therapist will be responsible for assessing patient needs, developing treatment plans, and collaborating with other healthcare professionals to ensure comprehensive care.DutiesConduct thorough assessments of patients' physical, emotional, and cognitive abilities to determine their needs.Develop individualized treatment plans based on assessment results and patient goals.Implement therapeutic interventions to improve patients' skills in daily living activities.Collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to coordinate patient care.Provide education and support to patients and their families regarding treatment plans and progress.Document patient progress and adjust treatment plans as necessary.Participate in discharge planning to ensure a smooth transition for patients from one care setting to another.Stay current with best practices in occupational therapy and maintain knowledge of medical terminology.QualificationsA degree in Occupational Therapy from an accredited program.Current state licensure or eligibility for licensure as an Occupational Therapist.Experience in inpatient, outpatient, pediatrics, geriatrics, school settings, or occupational health is preferred.Knowledge of discharge planning processes and familiarity with ICU settings is a plus.Strong communication skills and the ability to work collaboratively within a healthcare team.Proficiency in medical terminology related to occupational therapy practices.A compassionate approach with a strong desire to help others improve their quality of life.
