Home Health Care RN (Part Time)

Proactive Home Health Care is seeking a dedicated and experienced Home Health Care RN, Clinical Manager to oversee and manage the clinical operations of our home health services. In this leadership role, you will supervise Licensed Practical Nurses (LPNs) Registered Nurses (RNs) and (CNAs), ensuring the highest quality of patient care while maintaining compliance with regulatory standards. Key Responsibilities: Supervise and oversee the clinical team, including Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Care team and home health aides, ensuring patient care plans are implemented effectively. Ensure that comprehensive assessments are conducted, and individualized care plans are developed and executed to meet patient needs. Monitor the quality of patient care, providing guidance and direction to LPNs and other clinical staff. QA and auditing patient care plans and charts. Conduct performance evaluations, provide ongoing education, and ensure compliance with home health care regulations and policies. Coordinate care with the interdisciplinary team to ensure seamless communication and delivery of services. Maintain up-to-date documentation of patient care, interventions, and outcomes. Collaborate with physicians, therapists, and other health care providers to adjust care plans as needed for optimal patient outcomes. Qualifications: Active Registered Nurse (RN) license in Michigan. Minimum of 3 years of home health care experience, with at least 1 year in a supervisory or management role. Proven experience in overseeing and supervising Licensed Practical Nurses (LPNs). Strong knowledge of home health care regulations, policies, and CMS guidelines. Excellent leadership, communication, and organizational skills.
