We are launching the PRIME Access Model (Primary Care Integrated Model for Extended Access) in Weston - an innovative primary care structure designed to expand evening and weekend availability while preserving longitudinal panel continuity and physician sustainability. PRIME represents the next evolution of team-based, value‑aligned primary care.
PRIME is more than extended hours – it is a redesigned primary care operating model into an Always On Primary Care structure to meet our patients where and when they need us, integrating access, responsibility, continuity, and sustainability.
Schedule Structure
Day Shift: 7:00 AM - 2:00 PM
Evening Shift: 2:00 PM - 9:00 PM - THIS POST IS FOR THE EVENING SHIFT ONLY-
Rotational weekend coverage
Physicians participate in a structured shared‑shift partnership with defined handoff protocols to ensure continuity of care and reduced after‑hours burden.
Key Responsibilities
Shared accountability for a defined patient panel with shared Quality Incentive design
Proactive chronic disease management and preventive care gap closure
Structured in‑basket coverage across shifts
Same‑day and expanded access scheduling participation
Contribution to value‑based initiatives including AWV and TCM
Commitment to documentation excellence and quality performance

Qualifications
MD or DO
Board‑certified or board‑eligible in Internal Medicine or Family Medicine
Commitment to team‑based care
Interest in innovative primary care models
Strong communication and collaborative skills
