SummaryThe Licensed Master Social Worker is responsible for performing daily social work operations, providing direct patient care, and collaborating with clinical teams to deliver evidence-based behavioral health services.Key Responsibilities- Conduct patient assessments using evidence-based tools- Provide treatment for behavioral health conditions (CBT, Motivational Interviewing, crisis intervention, Interpersonal Psychotherapy, behavioral activation)- Make treatment recommendations during collaborative care meetings- Monitor treatment through care planning and coordination with clinical teams and health homes- Deliver crisis intervention and risk assessments as needed- Engage clients with limited behavioral health utilization through outreach efforts- Document services in a timely manner- Participate in weekly clinical supervision, team meetings, and case conferencesQualifications- LMSW licensure- Master of Social Work degree from an accredited institution- Proficiency in behavioral health screening tools and biopsychosocial assessments- Ability to utilize DSM-V diagnostic criteria for diagnosis and treatment- Experience assessing risk and collaborating with psychiatric and clinical teams- Ability to document services in electronic health records and review relevant health data- Strong communication skills for integrated care teams (PCP, NP/MD, Health Home Care Management staff)- Therapy experience required
