Great new Career Opportunity in Home Care!Now Hiring - Physical Therapist PRNThe Physical Therapist is responsible for, but not limited to:Provide compassionate, patient-centered care in the home, creating a positive and supportive experience during each visitConduct in-home assessments to evaluate patients’ mobility, functional abilities, pain levels, and overall safetyDevelop individualized treatment plans aimed at improving ADLs, strength, balance, and overall independenceDeliver hands-on therapy and interventions in the patient’s home using evidence-based techniques and appropriate assistive devicesEducate patients, families, and caregivers on exercises, mobility techniques, and safe practices to support progress between visitsTrack and evaluate patient outcomes, adjusting plans of care as needed to ensure the most effective resultsCollaborate with interdisciplinary team members to coordinate care and support smooth transitions between levels of serviceDocument all visits, treatments, and progress in compliance with agency standards and home care regulationsMaintain reliable and timely attendance to ensure continuity of care and a strong patient-provider relationshipEducation:Master's Degree in Physical TherapyProfessional Experience:Current Physical Therapist Certification in the state of Operations1+ years experience in Home Health, Acute Care setting or Rehabilitation Facility#APPLOIOTHER
