Care is provided under the direct supervision of a registered nurse in collaboration with the IDT and comprehensive plan of care to provide safe and effective patient care.Roles & ResponsibilitiesProvides direct care to patients following the home health plan of careSubmits accurate and timely nursing follow up assessment notes to organizationReports to RN any changes in patients conditionProvides routine and continuous level of care as ordered by IDTPerforms skilled instruction to patients and families regarding medication administration, management, and treatmentsProvides patients and families with emotional support and therapeutic communicationEncourages patients and families to participate in the home health plan of careProvides patient education regarding disease process, s/s of exacerbation and measures to promote comfortCoordinate care with other home health care providers including nursing, therapy and supportive services, such as CHHAProvides safe medication administration, reconciliation and home oxygen therapy management and educationPerformed duties as assigned by the registered nurse timely and accuratelyAttends monthly in-services and professional development opportunitiesFollows all agency policies and procedures, including in Infection Control, Emergency management, medication management, emergency management, pain management and overall patient care policies and procedures.Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policiesQualificationsCurrent LVN license issued from the California Board of Vocational Nursing and Psychiatric TechniciansAt least one year of nursing experience, preferably in home health, homecare or public health nursing.Has an active CPR CardKnowledge and compliance of professional nurse's act and standards of careMust have a valid and active drivers licenseMust have car insuranceLiability insurance is optionalMust have an automobile in good working conditionMust have great interpersonal and communication skillsMust be proficient in the use of computers for computerized clinical documentation
