The Active Lives Practitioner Post is an innovative post to deliver the National Physical Activity Standards. This post is based within the Dumfries North Home Team; a multi-disciplinary integrated team. Home Teams deliver a range of functions including rehabilitation and reablement with a key focus on Early Intervention and Prevention.We are looking for a motivated and enthusiastic individual with a Level 3 Diploma in Exercise Referral (CIMSPA recognised) who is keen to work within an integrated team alongside Active Communities and the wider Sport and Leisure teams.RoleThe post holder will require to take a lead role in the development and delivery of the Active Lives Pathway. Thus will include:Providing specialist programmes and advice, including onward signposting to enable people with complex needs including health conditions to access safe and inclusive community physical activity opportunities.Enhancing awareness of the health benefits of physical activityTo design, deliver and evaluate physical activity behaviour change programmes encourage and enable people who are physically inactive to become and stay active.A current driving licence is an essential due to the geographical area this team covers.
