Schedule: Full-Time, Monday–Friday (Flexible daytime schedule)
Salem, McMinnville & Eugene (home-visit model)
Salary: $96k - $100k
We’re hiring 2 licensed Physical Therapist for a reputed top home healthcare serving Oregon. This is a great opportunity for both experienced PTs and motivated new graduates who want autonomy, flexible scheduling, and meaningful one-on-one patient care.
Key Responsibilities
Conduct in-home PT evaluations and create individualized treatment plans.
Provide therapeutic exercises, manual therapy, functional mobility training, and patient education.
Document care in the EMR system within required timelines.
Collaborate with nurses, OTs, social workers, and physicians to support patient progress.
Travel between patient homes across assigned territory (mileage reimbursed).
Maintain HIPAA compliance, safety standards, and equipment sanitation during each visit.
Manage a dynamic caseload independently and adjust treatments based on patient needs.
Must-Have Requirements
Active Oregon PT License (Doctorate or Master’s).
Current CPR/BLS certification.
Reliable transportation, valid driver’s license, and auto insurance (daily travel required).
Open to new graduates; 1+ year home-health PT experience preferred.
Ability to work independently, manage time effectively, and complete accurate EMR documentation.
Eligibility to work in the U.S.
Professional references (2–3).
Pay: $96,000.00 - $100,000.00 per year
