Temp Per Diem Community Health Nurse - LHCSA

Position Summary: Provides skilled nursing care to patients in their home, including nursing assessment, interventions and supervision of Home Health Aides and Personal Care Aides in the field on a per visit basis.Principal ResponsibilitiesKnowledge, Skills And RequirementsPer diem RN Field Supervisor position available with a well-established Licensed Home Care agency in Nassau County. Duties include Patient Assessment, Reassessment and Aide supervision for patients receiving personal care and NHTD services.Perform in-home client assessments and/or RN supervisory visits for home care aidesComplete and submit all agency paperwork in a timely manner regarding visits to meet NYSDOH requirementsSupports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.Adheres to the organization’s policy in regards to absenteeism and appearanceSalary Range: $55-60 per /hour, $75-4100 per visit, commensurate with experience.Job Competencies & Minimum QualificationsKnowledge, Skills And RequirementsGraduate of an accredited school of nursing; Bachelor’s degree (B.A.) in Nursing from a four-year college or university preferred. One year of hands on nursing experience.Must have current license and registration to practice as a Registered Professional Nurse in New York State. One year Home Care experience preferred.Problem solving abilities while multi-tasking; ability to exercise sound judgmentExcellent verbal and written communication skillsExcellent customer service and organizational skillsWorking Condition/Physical DemandKnowledge, Skills And RequirementsExperience with a Clinical EMR, HHA Exchange experience preferredMust have a car and be willing to drive to Long Island client homesThis is fully field work role
