Remote Care Coordinator for Daviess County and Surrounding Areas in Indiana

Job OverviewJoin our esteemed care management team as a Care Coordinator, where you will have the opportunity to significantly impact the lives of older adults facing intricate medical and behavioral health challenges. In this remote role, you will be the primary care manager for a designated panel of members, guiding them towards improved health outcomes and enhanced quality of life.Key ResponsibilitiesServe as the primary care manager for members with elevated medical risks and coexisting behavioral health needs.Conduct comprehensive needs assessments through face-to-face interactions and telephonic communications, evaluating members’ medical, behavioral, functional, cultural, and socioeconomic factors.Formulate and execute individualized, person-centered care plans that reflect each member's objectives and readiness for change.Collaborate with internal teams, healthcare providers, and community resources to ensure the effective implementation of care plans.Facilitate appropriate referrals and connections to both internal services and community-based providers, including primary care physicians and specialists.Required SkillsStrong interpersonal and communication abilities.Proficient in conducting thorough needs assessments.Competent in developing and implementing tailored care plans.Familiarity with community resources and support organizations.Experience with behavioral health and socioeconomic determinants affecting health.Proficient in using Microsoft Office applications (Word, Excel, Outlook).QualificationsA Master's degree in Social Work, orA Bachelor's degree in Social Work, Psychology, Special Education, or Counseling, accompanied by a minimum of one year's experience in case management for older adults or individuals with disabilities or serious mental illness, orCurrent, unrestricted independent licensure as a Registered Nurse in Indiana, orA Licensed Practical Nurse (LPN) with three or more years of clinical experience working with older adults.At least one year of experience collaborating with local behavioral health providers and community support organizations addressing social determinants of health.A resident of Indiana with reliable transportation for travel within the designated area.Career Growth OpportunitiesWe are committed to recognizing and rewarding your performance in a nurturing environment that challenges you while providing clear paths to success.
