This role focuses on delivering safe, patient-centered care during overnight shifts, including medication administration, clinical monitoring, and coordination with the interdisciplinary care team.Key ResponsibilitiesAdminister medications and treatments as prescribed by physiciansMonitor residents' conditions and promptly report any changesPerform routine nursing assessments and accurately document care providedProvide wound care and other basic nursing procedures within scope of practiceMaintain complete and accurate clinical documentation and chartingAssist with admissions, discharges, and transfers as neededSupport Certified Nursing Assistants (CNAs) in resident care activities when requiredEnsure adherence to infection control and safety protocols at all timesRespond to emergencies and resident needs during night shiftsCollaborate effectively with the interdisciplinary healthcare teamRequired QualificationsActive and unrestricted LPN license (Colorado or compact state eligibility)Prior experience in long-term care or skilled nursing preferredAbility to work consistent overnight 12-hour shiftsStrong clinical judgment and documentation skillsAbility to work independently and manage patient care during night shiftsFor more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
