Housing Coordinator

Title: Community Housing CoordinatorLocation: Remote - Must reside within one of the following Southwest PA counties: Allegheny,Armstrong, Beaver, Bedford, Blair, Butler, Cambria, Fayette, Greene, Indiana,Lawrence, Somerset, Washington, WestmorelandMay have a routine onsite location like a CBO or health center, may need to meet a member in the community or attend community events from time to time but there is no office location. Outside of those, remote workRemote position with local travel required up to 75% (travel is reimbursable)• Schedule includes regular business hours Monday – Friday with occasional eveningor weekend availability for community eventsESSENTIAL RESPONSIBILITIES:• Research, evaluate and analyze demographic data to identify viable housing andutility support options for members.• Identify member barriers, develop action plans, and engage in empoweringinteractions to guide members towards sustainable housing options and/or utilityservice.• Act as a liaison for members, Case Managers and housing agencies in assistingindividuals and families with finding affordable, accessible housing.• Engage with members of the health plan face-to-face to support their housingneeds.• Utilize the health plan’s system of record to document all interactions with andrelated to the health plan members.• Serve as housing subject matter expert in affordable, accessible housing options.• Develop strong partnerships and collaborate with community organizations andvarious federal, state and local government agencies such as (HUD) and the statehousing authority in identifying and finding affordable housing options for members.• Participate in the development of programs that are effective, measurable andinnovative, with a special emphasis on reaching at risk members.• Represent the organization at community events, including events sponsored bycommunity partners.• Other duties as assigned and requestedEDUCATIONRequired• High School Diploma or GEDEXPERIENCERequired• 1 year in community outreach or customer service• 1 year working with at-risk populationsPreferred• Experience in Medicaid regulations• Experience in customer service/member services within the healthcare sector• Experience working with unhoused populations
