This PRN position offers flexible, as-needed scheduling while providing personalized, high-quality care to patients in their homes.Key Responsibilities:Provide skilled nursing care to patients in accordance with the plan of care and physician ordersMonitor patient health status and report changes to the supervising Registered Nurse or physicianAdminister medications and treatments as prescribedAssist with wound care, vital signs monitoring, and other clinical tasksEducate patients and caregivers on disease management, medications, and home care techniquesMaintain accurate and timely documentation in compliance with company policies and regulatory standardsCollaborate with the interdisciplinary care team to ensure coordinated patient careTravel to patient homes within the assigned service areaQualifications:Current Licensed Practical Nurse (LPN) license in the state of TennesseeCurrent CPR certificationValid driver's license, reliable transportation, and current auto insuranceStrong clinical and communication skillsAbility to work independently and manage time effectivelyPrevious home health experience preferred but not requiredWork Schedule:* PRN (as needed)* Flexible scheduling based on patient needs and coverage requirementsAbout All Heart Home Care: At All Heart Home Care, we are committed to providing compassionate, high-quality care to patients in the comfort of their homes.
