Administer medications, treatments, and routine nursing procedures as prescribed. Develop and implement personalized care plans in collaboration with healthcare teams. Educate patients and caregivers on health management, medications, and preventive practices. Coordinate care effectively with physicians, therapists, social workers, and other providers. Maintain accurate clinical documentation and patient records. Monitor patient progress and report changes in condition promptly. Ensure all procedures comply with healthcare regulations, safety standards, and organization policies. Required Skills & Qualifications Current Registered Nurse (RN) license in Massachusetts. Associate's (ADN) or Bachelor's Degree in Nursing (BSN). 1-3 years of experience preferred, particularly in community health, outpatient, long-term care, or similar settings. Strong clinical assessment, care coordination, and patient education skills. Knowledge of chronic disease management and preventive care. Excellent communication, organizational, and teamwork abilities.
