The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.ResponsibilitiesCoordinate and manage patient care plans for acute care settings. Assess, plan, implement, and evaluate patient care needs and services. Work collaboratively with the healthcare team to ensure quality patient outcomes. Document patient information accurately within the Cerner charting system. Provide education and support to patients and their families regarding care plans. Float to different units as required and participate in weekend and holiday shifts.QualificationsCurrent CA RN License with solid experience in Care Coordination. Prior experience with Dignity/Common Spirit facilities highly preferred. Basic Life Support (BLS) Certification required. LA City Fire & Safety certification required. Flexibility to work every other weekend and float to various units.
