Connecticut Licensed Physical Therapist Assistant
A company is looking for a Connecticut Licensed Physical Therapist Assistant (Care Navigator) - REMOTE.
Key Responsibilities
Provide remote monitoring services to support patients in their home-exercise therapy program
Direct and assist users in therapeutic exercise and functional training through a technology platform
Maintain records and provide ongoing feedback to clinicians on patient progress
Required Qualifications
Active Connecticut PTA license
Graduated from an accredited physical therapy program
2+ years of experience in musculoskeletal health is a plus
Tech-savvy, comfortable with multiple platforms (EHR, messaging, scheduling)
Experience in behavioral modification or Health Coach Certification is a plus
