This position is responsible for managing a defined territory and delivering comprehensive nursing services across the home health continuum. The role is 100 percent patient care and requires collaboration with team members to ensure continuity of services when covering for scheduled time off.Schedule & Work DetailsSchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Hour Guarantee: 32 hoursProductivity Expectation: 30 points per weekCoverage Area: Luzerne CountyOn‑Call/Weekend Requirement: Every third weekend and one night of on‑call coverage per weekMileage Reimbursement: Yes, reimbursed between patient visitsPay Rate: $51 to $53 per hourKey ResponsibilitiesServe as RN Case Manager for an assigned territory, providing direct patient care and maintaining accountability for assigned patientsPerform skilled nursing visits including routine visits, start of care, resumption of care, recertifications, and dischargesComplete comprehensive patient assessments and develop, implement, and update individualized plans of careCoordinate care with physicians, interdisciplinary team members, patients, and families to support optimal outcomesProvide patient and caregiver education related to disease processes, medications, and safetyMaintain accurate, timely documentation in the electronic medical recordSupport team coverage by assisting with visits when colleagues are off to ensure continuity of careAdhere to agency policies, infection control standards, and state, federal, and CMS regulationsRequired QualificationsActive Registered Nurse licenseAbility to work independently in a field‑based roleStrong clinical assessment, organization, and documentation skillsPreferred QualificationsRecent home health nursing experienceExperience using Homecare HomeBase (HCHB)Strong time management skills and adaptabilityPride-Health offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment OpportunityPride-Health is an equal opportunity employer.
