LPN Case Manager/Field Nurse Home Health Care

We are seeking a dedicated and skilled Home Health LPN Case Manager/Visiting Nurse to join our team. The ideal candidate will be responsible for reviewing medical records to ensure quality control and best practices in standards of care and making patient home visits when deemed necessary. Qualifications: Current LPN licensure in the State of Kentucky or a Compact State. At least two years related experience in home health preferred. Case Management and utilization review experience preferred. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job functions. Key Responsibilities: Develop and manage individualized care plans for patients. Conduct home visits to provide direct patient care and assess patient needs. Collaborate with physicians, RNs, and other healthcare professionals to ensure comprehensive care. Ensure compliance with all healthcare regulations and company policies.
