Occupational Therapist - Home Based Primary Care

This Occupational Therapist position is assigned to Home Based Primary Care.This assignment carries primary responsibility for providing in-home occupational therapy assessments, and the planning, administration and management of rehabilitation in the home for patients of the Home Based Primary Care Program (HBPC). The OT will provide these in-home services to patients with a wide variety of multiple, complex chronic illnesses and/or profound disabilities requiring knowledge of contemporary and evidence based Occupational Therapy practices. Provides professional independent services for both assessments and development of patient treatment plans. Decisions are made in the community without onsite supervision of another health care professional.Duties may include:Independently develop, implement and evaluate therapeutic rehabilitation programs within the framework of HBPC and Physical Medicine and Rehabilitative Service (PM&RS).Assess the goals, living skills, physical abilities, and related needs for each patient referred for in-home assessment.Develop a treatment plan directed toward achieving maximum level of functioning within the home, consulting with other PM&RS disciplines as indicated, and developing the plan in collaboration with the patient and caregiverImplement the treatment plan by: providing direct patient care; procuring necessary equipment/adaptive devices; instructing and training the client and caregiver concerning exercises, activities of daily living, rehabilitation techniques, and equipment/adaptive devices; and recommending community referrals. Reassess patients' levels of functioning and home environment, revising the treatment plan as indicated and terminating treatment objectives when met.Document initial assessments, treatment plans, and response to treatment as determined in subsequent patient home visits, telephone calls, and case management work, according to HBPC policy and JC standards.Complete Encounter information in accordance with Truman VA Medical Center guidelines.Using the computer system for storing and retrieving electronic documents and files, transmitting and receiving e-mail, activating printers, requesting leave, and accessing the Computerized Patient Record System (CPRS)Communicate and coordinate coverage for anticipated leave and communicate unanticipated leave in accordance with PM&R Service policies.Participate in the HBPC Performance Improvement Program, including completing assessment and documentation required by performance measures applicable to the OT's role.Demonstrates advanced competency in knowledge of electronic documentation and computer skills.Work Schedule: Compressed, Monday-Friday, 7 am - 530 pm.
