Key Responsibilities Coordinate inpatient care transitions, discharge planning, and community resource linkage for diverse patient populations. Collaborate with MDs, RNs, and therapy teams to optimize LOS, ensure medical necessity, and execute safe handoffs. Manage utilization review, payer authorizations, and compliance with CMS/InterQual criteria using Epic workflows.
