LPN – MDS Nurse Rate: $30 - $33/hr. (depending on experience) Shift: Full-Time Shape care plans. Improve outcomes. Make a difference. At Seneca Health Care Center, we’re seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to join our team as an MDS Nurse. In this role, you’ll help ensure residents receive the right care at the right time—by using your clinical knowledge and documentation skills to support accurate assessments and individualized plans. If you’re an LPN who enjoys working with data, collaborating across disciplines, and making sure every box is checked for better care, this is the perfect opportunity. Your Role Includes: Completing and coordinating timely MDS assessments and submissions Collaborating with the interdisciplinary team on resident care plans Ensuring documentation meets regulatory and quality standards Tracking resident changes and updating care plans accordingly Supporting audit readiness, case-mix accuracy, and survey prep What We’re Looking For: Valid NYS LPN license Experience with MDS (preferred) or willingness to learn Strong attention to detail and clinical judgment Excellent communication and teamwork skills Prior experience in skilled nursing or long-term care a plus Why join our team?
