If you are unable to complete this application due to a disability, contact this employer to ask for an accommodation or an alternative application process.HDM Program AssistantRegular Full-Time San Francisco, CA, US30+ days ago Requisition ID: 2118Salary Range: $20.50 To $22.50 HourlyTitle: Home Delivered Meals Program AssistantDepartment: Nutrition and Senior CentersFLSA Status: Non-ExemptReports To: Home Delivered Meals & Transportation Program ManagerSummary: Assists the manager and coordinator in daily Home Delivered Meal program operations.Essential FunctionsConducts HDM consumer intake according to waitlist policies and procedures.Conducts in-home assessments for seniors to evaluate their need for HDM and their functions in various areas.Inputs HDM client personal data and program data in Get Care.Conducts in-home and telephone consumer satisfaction surveys.Assists the manager and coordinator in collecting program statistics and preparing reports.Assists with the training of workers and drivers.Attends in-service training and meetings.Maintains program records and reports for audit purposes.Works as a backup driver/worker, delivering meals to clients and meal sites as necessary.Performs other duties as assigned.QualificationsA college graduate with one year of work experience is required, or a high school graduate with 3 years of experience working with older adults is preferred.Must have and maintain a valid CA driver's license—minimum of two years of driving experience with a satisfactory driving record.Must be interested in working with seniors and adults with disabilities—the ability to work in a team environment.Bilingual in English and Chinese.Good communication, telephone, and organization skills.Must pass a drug test if assigned as a backup driver.Must pass a strength test for physical fitness for the job: the ability to lift up to 30 pounds.Self-Help for the Elderly is an Equal Employment Opportunity/Affirmation Action Employer and we welcome diversity in the workplace.
