LVN Case Manager and Discharge Planner

Job Description POSITION SUMMARY *Candidate must be a Licensed Vocational Nurse. The Discharge Planner works under the direction of the Facility Administrator, they will implement re-entry care plans and provide ongoing support to individuals served in accessing community physical and behavioral health resources, substance abuse treatment, housing, employment, education assistance, and other relevant needs to individuals who are incarcerated. This position includes an additional 5% weekend differential. JOB RELATIONSHIPS REPORTS TO: Clinical and Administrative, direct supervision provided by Facility Coordinator SUPERVISES: None DUTIES/RESPONSIBILITIES The Case Manager and Discharge Planner is responsible for coordinating pre-release or post-release care management and assessing the discharge planning needs of all patientsThe Case Manager and Discharge Planner will also coordinate community services to ensure successful re-entry into societyProvides case management and discharge planning services for individuals, including pre- and post-release interviews and coordination of community resourcesEnsures continuity of care by coordinating medical benefits, social services, and outside behavioral health/substance abuse services, as well as obtaining medication scriptsMay assist in the job shadow portion of the interview process and orientation/training of new employees.Documents relevant clinical information and data for quality initiative purposes to improve patient careIndividuals in this position will have skills to actively listen and demonstrate understanding to best meet the patient's individual needs.Due to the unique challenges of the target population individuals serving in this role will need good problem-solving skills.Collaborates with community providers to provide ongoing support to all individuals served in the areas of behavioral health resources, substance abuse treatment, employment, social relationships, education assistance, and other relevant needs Upon receiving the necessary training and/or instruction, performs other related duties as required or assigned. EDUCATION/EXPERIENCE REQUIREMENTS:A minimum of 2 years of experience and/or a bachelor's degree in social work, nursing or related field or equivalent combination of education/related expenses.
