Required Skills & Qualifications:Active Registered Nurse (RN) license in New Hampshire.Associate's degree (ADN) or Bachelor's degree (BSN) in Nursing.1-3 years of nursing experience preferred, especially in community health, outpatient, long-term care, or home health settings.Strong clinical assessment skills and the ability to coordinate care effectively.Knowledge of chronic disease management and patient education techniques.Excellent communication, organizational, and teamwork skills.
