Responsibilities/Job Description:Fairview is seeking an Internal Medicine physician to join our primary care practice at M Health Fairview Clinic Brooklyn Park.Practice Details:16 provider primary care practice with more than 12 specialists onsite.Patient encounters average 18-22 patients per day.34 patient contact hours per week.Call Frequency: approximately 1 per month - phone consultation only with first tier nurse triage.Openings: 1.0 FTE (Full Time) or less - Flexible schedule.As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.Fully integrated Electronic Medical Record (EPIC).Community Information:Brooklyn Park, Minnesota is a diverse suburban city northwest of Minneapolis and the largest city in Hennepin County.
