We are seeking a compassionate and dedicated Caregiver to join our team in Northeast Philadelphia, providing exceptional support and personalized care to individuals in various living environments. As a Caregiver, you will be instrumental in promoting independence, safety, and comfort for our residents and clients through attentive, respectful, and professional caregiving.
Duties
Assist residents and clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patients’ vital signs and observe for any changes in health status, reporting findings accurately to healthcare professionals
Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
Administer medications following prescribed care plans while adhering to HIPAA regulations for patient privacy and confidentiality
Support residents with memory care needs such as Alzheimer’s disease or dementia through engaging activities and consistent routines
Operate assistive devices such as Hoyer lifts and ensure safe transfer procedures to prevent injury
Maintain a clean, safe environment by performing light housekeeping tasks including cleaning patient areas and common spaces
Document care provided accurately in compliance with healthcare standards and facility policies
Collaborate with healthcare teams, social workers, and family members to develop and implement personalized care plans
Provide emotional support and companionship to enhance residents’ quality of life
Requirements
Proven experience in home health, assisted living, nursing homes, or group homes preferred
Knowledge of patient observation techniques, medication administration, and vital signs monitoring
Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus
Experience working with individuals with developmental disabilities, autism, Alzheimer’s or dementia is highly valued
Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
Strong communication skills to effectively interact with residents, families, and healthcare teams
