Accela Rehab & Care Center at Somerton has a great PRN opportunity for a Speech-Language Pathologist!Skilled nursing experience preferred!Duties and Responsibilities:Assessment of referred patients that require rehabilitation servicesProviding direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitationEstablish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the PhysicianSet realistic and achievable goals for their patientsDocument and record the patients’ conditionEducate patients and families in an appropriate therapy methodEvaluations, weekly progress notes, recertification, and discharge documentationsCollaborates with all disciplines to provide best patient centered treatment and carePerform all other duties assigned by Director of RehabilitationCompliance with confidentiality, HIPPA and Federal Residents RightsQualifications/Requirements:Must have graduated from an accredited Speech Therapist college or universityMust have a valid professional license and registration for state of PAExperience with long-term care and/or geriatric care is preferred but not limited toMust have ASHA CertificationPlease submit a resume and apply for more information.
