Operation Breakthrough strives to support and empower the children's families through advocacy, referral services and emergency aid.Job SummaryThe Clinical Case Manager works collaboratively with an interdisciplinary team to support caregivers in meeting their mental health, functional, and family-stability goals. This role provides short-term clinical intervention, care coordination, and crisis support while helping families navigate complex systems and access the resources they need.Minimum Qualifications:Master's Degree in Social Work, Marriage and Family Therapy, Professional Counseling or related fieldKnowledge of the clinical impact of trauma, attachment, diversity, and povertyPrevious experience in case management, social services, behavioral health, or working with families in high-need communitiesDemonstrated ability to provide trauma-informed, culturally responsive supportStrong communication, organization, and time management skillsAbility to work both independently and collaboratively in a fast-paced environmentDemonstrated knowledge of community resources and ability to support families in addressing social determinants of healthPreferred (but not required):Clinical licensure or working toward licensureExperience in medical social work or behavioral health settingsEssential Responsibilities:Collaborate with Family Advocates, Adult Therapists, educators, and interdisciplinary team members to support families holisticallyProvide short-term individual clinical case management to caregivers seeking mental health supportWork alongside Family Advocates to take on and manage high-acuity Care Coordination cases, providing direct support to stabilize families and guide them through crisis until they transition back to ongoing supportProvide intervention and support during periods of high acuity, addressing functional, behavioral, and mental health needs while navigating multiple systemsRespond to client crises and provide therapeutic support, including assessment and intervention related to suicidal risk, behavioral challenges, and other urgent needs, ensuring appropriate resources and warm handoffsFacilitate psychoeducational and support groups for caregivers and support parent engagement initiativesAdvocate for families by assisting with access to resources and navigating systems such as healthcare, housing, and educationMaintain timely and accurate documentation in the EHR and other agency systems, including intakes, assessments, progress notes, and treatment plansTrack client progress and outcomes to support program evaluation and continuous improvementParticipate in weekly interdisciplinary team meetings and consultations (child, adult, speech, occupational, and music therapy) to support a collaborative and accountable team cultureBuild and maintain partnerships with community organizations to expand accessible services for familiesServe as the point of contact for higher education institutions regarding practicum and internship students, including onboarding and coordination with supervising staffMaintain appropriate professional boundaries and adhere to ethical standards of practiceParticipate in weekly supervision (agency and clinical, as applicable) and maintain licensure requirementsEngage in ongoing professional development opportunitiesAdditional Expectations:Maintain flexibility to meet the needs of families, including occasional evenings or participation in family engagement eventsSupport a culture of equity, inclusion, and respect in all interactions with families, staff, and community partnersAbility to respond effectively in crisis situations and adapt to changing needsWORKING CONDITIONSIndoor office environment.This job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that may be required in this role.Operation Breakthrough is an Equal Opportunity Employer.
