Job Description:Physician - Primary Care / Post-Acute Care (Warwick, RI)SIGN ON / RELOCATION BONUS NEGOTIABLEJob Type: Full-time or Part-timeSetting: Skilled Nursing Facilities, Assisted Living Centers, Memory CareLocation: Warwick, Rhode IslandAbout the RoleWe are seeking a dedicated Primary Care Physician (Internal Medicine, Family Medicine, or Geriatrics) to provide high-quality patient care across Skilled Nursing Facilities, Assisted Living Facilities, and Post-Acute Care settings. This is a unique opportunity to join a structured care model that delivers primary care directly to patients in their living environment.The incoming physician will enjoy flexible scheduling, independence, and the opportunity to take on Medical Directorships across multiple facilities.Key ResponsibilitiesProvide face-to-face primary care services within skilled nursing and assisted living settings.Manage acute, chronic, and post-acute medical conditions among geriatric and rehabexams, manage medications, order labs/imaging, and coordinate care.Collaborate with nurses, NPs, and PAs to ensure seamless patient care.Oversee 5-8 assigned facilities, visiting each twice per week.Serve as Medical Director for assigned facilities (optional additional income).Participate in limited rotation-based call coverage (with option to sell call shifts).Schedule & Call DetailsMonday to Friday - Round-and-Go modelNo shift work; visit your facilities and complete documentationOvernight, weekend, and holiday coverage handled by outsourced tele-service2-5 buildings without tele-coverage rotate call once a monthPatient VolumeAverage: 15-20 patients per dayBenefits (W-2)3 weeks vacation2 weeks sick time6 paid holidaysMedical & Dental insuranceOccurrence-based malpractice401(k) retirement planSign-on bonus: negotiableQualificationsMD or DO with training in Family Medicine, Internal Medicine, or GeriatricsActive or eligibility for Rhode Island medical licenseDEA licenseExperience in skilled nursing or post-acute geriatric care preferredNew graduates considered if exposure to geriatrics/Post-Acute CareMust be comfortable working collaboratively and independently
