General Overview Physician Affiliate Group of New York, P.C. (PAGNY) for the Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for delivering medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system receive care on the mental health service. This service strives to provide safe, holistic and evidence‑based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. It provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24‑hour on‑call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re‑entry services, and relationships with multiple training institutions throughout the city. Under supervision of the Site Medical Director with broad latitude for independent judgement and initiative, the Staff Physician is an integral part of the medical team at his/her facility. The Staff Physician is expected to take a leading role in the delivery of health care services to our patients and is responsible for the supervision of the Physician Assistants and Nurse Practitioners. Responsibilities Complete comprehensive histories and physicals on all new admissions, including the master problem list (MPL) with diagnosis, lab, and EKG contraindication form and medication list where applicable. Perform S.O.A.P. notes on patients requesting sick call, schedule follow‑ups, and update medication orders. Update any deficiencies on the MPL. Diagnose and treat acute and chronic illness, evaluate the need for consult services, and submit the prioritized consult when indicated. Review all on‑island/off‑island specialty consult clinic and hospital returns, evaluate the recommendations to ensure that the standard of care is met, and implement the recommendations of the consultant. Respond to emergencies and fire drills on a priority basis in a timely and professional manner. Be familiar with performance indicators (PI) and take appropriate measures to comply with these standards. Perform chart reviews and summaries on all transfers, update the MPL, rewrite medication orders, and resubmit consults as needed. Request radiology exams, lab tests, EKGs when indicated and interpret these results in conjunction with the patient’s clinical status. Notify Urgicare for 3‑hour/EMS runs. Complete and submit all encounter forms on a daily basis. Witness, sign and document patient healthcare refusals. Print name or use a name stamp. Write legibly. Generate medical modifications as needed. Generate special needs tracking form as needed. Maintain clinical competency by reviewing PHS protocols, current medical literature and C.M.E.s. Provide clinical supervision for P.A.s/N.P.s, including co‑signing of notes and medication orders, and ensure that the standard of care is maintained. Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed. Maintain your tour schedule as directed with particular attention to punctuality and timely notification of absences. Maintain required credentials and current licensure and CME requirements (appropriate documentation must be on our files). Maintain a professional attitude and appearance. Attend all mandatory in‑services. Comply with and maintain Employee Health requirements. Qualifications Medical Doctor with 3 to 5 years of directly related experience, including residency in a directly related medical specialty. Registered to practice medicine in the State of New York. Federal DEA, BCLS or ACLS, and Infection Control Certification. Medical Specialty License or Certification. Board certified or Board Eligible in specified area of medical specialty.
