Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel.Essential FunctionsConsults with and keeps supervisor informed of unit activities, needs and problems related to patient careProvides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shiftTakes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner.Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in roomMaintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lightingAssists in patient admission, transfer and discharge proceduresAssist with handling and care of patient belongings and other personal propertyAnswers patient call lights, telephone, and paging system and responds as appropriateAble to release, remove and reapply restraints to patient under the direction of an RNEnsures patient is in proper position and alignmentAssist RN and/or LVN/LPN with complicated patient treatment proceduresServes and removes patient meal tray in timely mannerAssists with feeding and/or preparing items such as opening mild container, cutting food for patientAssists patients with range of motion exercises, and movement to wheelchair or activity areasConducts job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standardsDocumentation via electronic health recordAttend interdisciplinary meetings as needed to provide input into selected patient's plans of careDocuments restorative services provided following facility policies and proceduresKnowledge/Skills/Abilities/ExpectationsExcellent oral and written communication and interpersonal skillsAbility to react decisively and quickly in emergency situationsAbility to communicate effectively with patients and their family members, and facility staffBasic computer knowledgeApproximate percent of time required to travel: 0-10%Must read, write and speak fluent EnglishMust have good and regular attendancePerforms other related duties as assignedQualificationsEducation* High school diploma or equivalent preferredLicenses/Certification* Valid certification as a CNA in the state of employment* CPR certification requiredExperience* Six months experience in a long-term care environment preferred
