This position supports patient care during staff absences or open positions, delivering skilled physical therapy and case management as needed. Through clinical expertise, collaboration, and adaptability, Float PT ensures that every client receives the care and attention they deserve, no matter where they live.As a Float Physical Therapist, You WillProvide Direct Client Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders. Deliver high-quality care that empowers clients to achieve their functional goals. Maintain Accurate Client Records and Billing – Document all services accurately and promptly using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.Case Management & Continuity of Care – Serve as primary client coverage for staff members on FMLA, sick leave, vacation, or during open positions. Manage cases as needed based on duration of coverage, including all visit types and coordination with the interdisciplinary team. Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. This Job Also RequiresCurrent Physical Therapist license in the state of Minnesota. CPR certification required within 6 months of hire. Willingness to travel across the region as necessary.
