Behavioral Health Case Manager - Intake Coordinator
Job Title: BEHAVIORAL HEALTH CASE MANAGER - INTAKE COORDINATORReports To: Behavioral Health Program SupervisorScheduled Hours: Full-Time (37.5 hrs. per week)FLSA Status ExemptPurpose of Position:The Intake Coordinator, in coordination with the BH Program Supervisor, is responsible for determining weekly intake availability, communicating directly with ISK and MRC regarding intakes and scheduling capacity, maintaining a reduced case management caseload, and informing the BH Program Supervisor of any constraints or opportunities identified within the intake process.Essential Functions:Perform intake assessments with all new referrals to the Milestone Case Management program. This will include developing a basic understanding of the clients’ current situations and assigning an appropriate case manager.Carries a smaller caseload for the BH and/or NFM programs. At the direction of the Behavioral Health Supervisor, offers community-based BH services to consumers referred by ISK and/or offers nursing facility-based NFM services to those identified by an OBRA assessment as needing mental health intervention.Case manages the consumer’s overall delivery of service as stated in the Individual Plan of Service to ensure the comprehensive meeting of the consumer’s total unmet needs. This may include transporting as clinically indicated for linking and coordination of care in the employee’s vehicle.Collaborates with the appropriate medical, psychiatric in-patient, and/or ISK staff to facilitate the consumer’s discharge and safe return to the community from psychiatric hospitalization, medical hospitalizations, nursing homes, or to other less restricted environments.Provides consultation and education to family members of consumers to facilitate their understanding of the primary consumer’s needs and assist them in improving or preserving their involvement and relationship with the consumer.Provides case consultation, staff education, training, and support to AFC staff, nursing facility staff, or other involved service providers to increase their awareness of the consumer’s physical and mental health needs related to planned services.Completes all documentation in a timely manner as required by funders and regulatory agencies.Interacts with consumers served, legal representatives, natural supports, AFC staff, nursing facility staff, referral agents, colleagues, funder sources, and other community agents in a positive and professional manner.Participates in all required team and agency meetings, in-services, and trainings.Maintains professional Continuing Education requirements and State of Michigan licensure.Complies with all federal, state, licensing, agency, and funder requirements.Work directly with the Supervisor and program managers to align training efforts with performance expectations and program goals.Ensure that the new hire training aligns with the team's objectives, including caseload management, documentation quality, and customer service.Decision Making Authority:Most decisions are made by the incumbent in conjunction with established guidelines from the Clinical Manager of Mental Health Services, Integrated Services of Kalamazoo guidelines, and established MDHHS program policies and procedures. Routinely consult with Clinical Manager of Mental Health Services regarding overall program success and areas of concern.Education/Experience:A minimum of a Bachelor’s degree in a human service field plus current state certification or licensure in one of the following Michigan Licensures: RN, LLBSW, LBSW, LLMSW, LMSW, LLPC ,LPC, LLP, LP.A minimum of a bachelor’s degree in a human services field and two years of experience with people with mental illness.Preference will be given to persons with a combination of mental health experience and working with the elderly for at least two years.A minimum of two years of experience in a Case Management role with individuals who have a mental illness.Experience in evaluating the needs of individuals with mental illness.Previous experience in person-centered planning.Competencies:Communication Skills: Strong verbal and written communication skills to work with new clients and to coordinate with ISK, MRC, and other community partners.Organizational Skills: Ability to manage the intake schedule, including rescheduling no-shows, accommodating urgent requests from ISK, MRC and other community partners.Adaptability: Ability to adjust the schedule and processes as needed to accommodate our client cohort.Problem-Solving: Ability to identify challenges new clients face in gaining access to Milestone Case Management and develop strategies to address any issues identified in our internal processes, or how we coordinate with ISK, MRC and other community partners.Compensation & Benefits Exempt position with starting salary from 43k to 64k , based on license and experience levelBenefits packages include:Medical, dental, vision403b retirement plan with matchingShort term disabilityLife insuranceEmployee assistance programs (EAP)Robust Paid Time Off (PTO) accrualFlexible scheduling to promote a great work/life balanceWho is Milestone Senior Services?Our mission is to provide options that empower people to thrive in their community.Core Values: Collaboration, Dedication, Resourcefulness, Inclusivity, and Integrity.Milestone Senior Services (formerly Senior Services, Inc.) has been delivering to the Kalamazoo and surrounding communities for over 50 years.
