Reporting to the Nurse Cohort Manager, this role is responsible for delivering consistent, patient-centered care, maintaining excellent clinical documentation, and collaborating across care teams to ensure continuity and quality outcomes.This is a fully remote role designed for nurses who thrive in a fast-paced, mission-driven virtual care environment.Schedule & Employment TypeFull-Time- 36–40 (Option for 4, 6, 8, and 10-hour shifts)Key ResponsibilitiesClinical Care & Patient SupportDeliver virtual nursing care using evidence-based clinical judgment and the nursing processConduct comprehensive health assessments, triage patient concerns, and provide clinical guidance across multiple care settingsPromote patient health, safety, and wellness through proactive outreach and educationProvide education and clinical support to family and professional caregiversCommunication & Care CoordinationBuild strong rapport with patients, caregivers, care coordinators, and partner organizations in a virtual environmentCommunicate clearly, compassionately, and professionally with all stakeholdersIdentify and elevate urgent or concerning clinical findings according to established protocolsCollaborate with internal care teams to ensure timely follow-up and continuity of careDocumentation & QualityMaintain accurate, timely, and thorough documentation in the electronic health record (Athena)Ensure documentation meets PocketRN clinical standards, quality expectations, and regulatory requirementsAdhere strictly to HIPAA and patient privacy standards in all virtual interactionsProfessional Practice & Team EngagementFollow PocketRN workflows, clinical guidelines, and escalation pathwaysActively participate in coaching, performance feedback, and ongoing educationContribute to continuous improvement initiatives and support a collaborative team cultureDemonstrate accountability for schedule adherence, attendance, and professional conductQualificationsEducationGraduate of an accredited school of nursingCurrent BLS certificationExperienceMinimum of 3 years of direct patient care experienceExperience in geriatrics, primary care and dementia care strongly preferredExperience in telephone triage is preferredLicensureCurrent, active, and unencumbered RN license requiredLicensure in all 50 states requiredSkills & CompetenciesStrong clinical judgment, critical thinking, and assessment skillsExcellent verbal, written, and asynchronous communication skillsAbility to deliver empathetic, patient-centered care in a virtual settingComfortable working independently while remaining engaged with a remote teamAdaptable and eager to learn in a fast-paced health-tech startup environmentTechnical & Work Environment RequirementsProficiency with EHR systems (Athena preferred), Apple/macOS proficiency and Google Workspace toolsReliable high-speed internet connection (minimum 50 Mbps).
