Role Description This full-time role as a Long-Term Care (LTC) Consultant Pharmacist is based on-site in Leawood, KS. The Consultant Pharmacist will be responsible for optimizing medication therapies, ensuring regulatory compliance, and collaborating with interdisciplinary teams to enhance patient care in long-term care settings. Additional responsibilities include conducting comprehensive medication reviews, developing care plans, educating healthcare staff on medication-related issues, and providing recommendations to improve therapeutic outcomes. Qualifications Proficiency in Medication Therapy Management (MTM) and medication regimen reviewsExpertise in clinical pharmacy, including knowledge of chronic disease management and geriatric careRegulatory knowledge related to long-term care facilities and pharmacy operationsStrong communication and collaboration skills for interactions with interdisciplinary healthcare teamsDetail-oriented with skills in data analysis to identify medication-related opportunities for improvementDoctor of Pharmacy (PharmD) degree or equivalent, with active pharmacist licensureExperience in long-term care or consultant pharmacy preferredCommitment to providing high-quality care and enhancing patient outcomes
